During the initial call or when you first visit us, we will ask you for what seems to be a lot of information and questions about the deceased. We understand that, especially in the case of an at-need, it can be difficult for you to answer these as you can still feel many upsetting and strong emotions. It's important that you are comfortable and able to move at a pace that is best for you, so if you need to take a break, by all means, ask us, and we can resume when you're ready. Below you'll find a list of some of the basic information we request during this time.
Statistical Information - Statistical information includes a variety of information from basics like their full name, address, birth date to more in-depth like their occupation, father and mother information and more. Why do we ask for this information? Having this on file will help us fill out any and all provincial and federal forms or documentation as well as help us create their Book of Memories webpage. This information can be completed online or filling out the information on a spare piece of paper to bring to the appointment.
Social Insurance Card & Care Card Number - The deceased's social insurance card will be needed to help verify all important documentation required to move forward with burial or cremation.
Family Contact Information - The most important family contact information for us to have will be the next of kin information as well as anyone else involved in planning the arrangements.
A Photo - This photo will be used for a Book of Memories webpage, an obituary, and preparations.
Mementos
- Anything you think you would like to either include in the cremation (if possible) or burial or to put on display during the visitation or funeral.
Memorial Society of BC Membership Number (If applicable)
If you or your loved one has pre-arranged or pre-paid the arrangements then this information has already been gathered.
Identification - You may be asked to identify the family member that has passed and for a copy of the will to verify the executor.
If arrangements we're not pre-planned ahead of time, it will be up to you to make decisions about the disposition method (burial or cremation), selecting merchandise like urns, keepsake urns, caskets, or cremation caskets, whether or not you would like to have a viewing, what kind of services you would like to have, and more.
